You're using an old issue template

The PR template is very verbose. I think we can make it more concise. A simple checklist based on the CONTRIBUTING.md requirements would be appropriate, plus a bullet point list of what the PR does. It would be nice if we're able to use the template to apply tags - this might need some more advanced configuring w/ a bot, though. (E.g. apply tags based on the package(s) that are affected, bug, feature, etc.)
