UI updates (primarily grafana)

• Upgraded Grafana version
• Removed grafana_vol, migrated internal DB to use postgres, migrated remaining config to json provisioning files
• Consolidated widgets into a single dashboard for a cleaner and hopefully more reliable interface on ANMS UI Monitor page
• Added a prototype "Help" page to UI with some quick links that we can expand upon in time
• Added amp-manager to status page
• Created an UPGRADING.md file to document upgrade instructions going forward. For the grafana update, if using an existing database, a manual step is required to create the new empty grafana_internal_db

Note: Any other grafana plugins we may want (ie: those still on the other open grafana branch) will need to be replicated into this branch using provsioning files.

This PR addresses #283, #280, and a start on #268
