Bug: Duplicate entries in waiting lists

Description of the issue 📄

We occasionally get duplicate entries showing up in the waiting list. The first screenshot below is from a London workshop and shows the same name appearing four times. I would expect that each entry in the waiting list would be unique.

Note that they are not multiple people co-incidentally signing up with the same name. Every entry points to the same user. These duplicates are also counted in the summary at the top of the workshop admin page as shown in the second screenshot below.

This has happened in the past and I have reported it in the organisers channel in Slack. I don't recall how (or if) those instances were resolved. The fact that I cannot find it in the 90 day Slack backscroll shows that it's pretty infrequent.
